10/13/2023 EnchantaVaughn Approved Behavior: QMHP-A arrived to the home of Ms. Cosby. Upon arrival to the home, Ms. Cosby appeared to be in a positive mood evident by her facial expressions such as a smile on the face. QMHP-A inquired about her day. She explained to QMHP-A that her day was going well. QMHP-A informed Ms. Cosby that this session was going to focus on her independent living skills and the stressors that are associated with causing her anxiety and depression. Upon arrival, she did not present any symptoms of S/I. Intervention: QMHP-A began the session by inquiring about Ms. Cosby’s current thoughts, feelings and emotions. QMHP-A then inquired about her medication management and the importance of her adhering to her medication on a consistent basis. QMHP-A assisted Ms. Cosby with independent living skill exercises. QMHP-A accommodated Ms. Cosby with information that she will benefit her to become self-sufficient. QMHP-A conducted a skills building activity. QMHP-A explained to her the purpose of this activity was to assist her with different techniques of budgeting and saving. QMHP-A described to Ms. Cosby adult daily needs that will satisfy a single adult for the month or until necessary funds are available. QMHP-A reviewed Ms. Cosby ADL’s and encouraged Ms. Cosby to stay on task. QMHP-A assisted Ms. Cosby on several calming techniques. QMHP-A reviewed Ms. Cosby’s responses to her stressors. QMHP-A questioned Ms. Cosby about different ways she think is best way to respond to her stressor. QMHP-A engaged Ms. Cosby in a nature walk. QMHP-A questioned Ms. Cosby about the different sounds, smells, and visuals. QMHP-A conducted a skills building activity. QMHP-A informed Ms. Cosby that the purpose of this activity was to assist Ms. Cosby with being able to relax and clear her mind off negative thoughts and focus on positive thinking. Response: Ms. Cosby appeared to be receptive to QMHP-A’s interventions and services that were provided to her during the session. Ms. Cosby expressed to QMHP-A that she has not been on her medication on a daily basis because everything going on is making her losing focus due to everything going on. Ms. Cosby expressed to QMHP-A that she has been working on different ways to save her money so that she will not become depressed throughout the month. Plan: QMHP-A will teach the client relaxation skills (e.g., progressive muscle, guided imagery, slow diaphragmatic breathing) and how to discriminate better between relaxation and tension; teach the client how to apply these skills to her daily life. (1-2x weekly).
